Qualification procedure and scheduling your vasectomy.

- First, arrange the date of your vasectomy by phone (+48 607 776 777).
- Shortly after scheduling your vasectomy you will receive a text message with a link to an online health questionnaire. Fill in the questionnaire as soon as possible after scheduling the procedure. The questionnaire is part of the qualification process for vasectomy. Since not every patient qualifies for a vasectomy – as with any surgical procedure – the medical records must contain a justification for performing it.
- After completing, confirming and submitting the questionnaire, you will receive the informed request and consent form for the procedure at the e-mail address you provided. Read it calmly at home. You will sign this document before the procedure in the presence of our staff.
- After reviewing your questionnaire, Dr Kulik (the physician performing the vasectomy) will contact you by phone (usually 1-3 days before the procedure, or earlier if any additional tests or clarifications are required). This is the pre-operative telephone consultation
- You arrive for the procedure on the scheduled date.
The telephone consultation and the completed health questionnaire form part of the medical interview and are an integral part of the qualification process. That is why you must answer all the questions it contains.
A vasectomy, although minimally invasive and low-risk, is formally a surgical procedure and, like any other surgical intervention, requires that the benefit it brings outweighs the associated risk.
